FINANCIAL AID APPLICATION
Miami FC Breakers recognizes resources are limited for some families and we pledge to offer as many fundraising opportunities (with funds going towards player fees) during the year as possible. Families who need additional assistance may apply for financial aid in the form of extended payment terms, or scholarships.
However, we have a limited amount of funds available to be awarded. Note: To maintain armature status, the guidelines restrict financial aid or scholarship awards to assist with covering training fees and club registration/commitment fees.The reimbursement of travel expenses, elective team tournament fees, or uniform payment is not permitted.
***All Miami FC Breakers players are required to pay $150.00 nonrefundable deposit to be considered for Scholarship which will be credited to your club fees***
In order to be eligible for financial assistance/scholarship, a player must meet and adhere to all guidelines set forth in the Player Commitment Document. Failure to do so may result in the forfeiture of any financial assistance/scholarship awarded. The Financial Aid/Scholarship Application must be filled out completely and attached to a fully completed IRS 4506-T Form in order for your request to be considered. The deadline for submitting completed Financial Aid/Scholarship Applications for consideration for the 2018-19 soccer year is 7 days after your player is registered.
The Scholarship/Financial Assistance Committee will review your application and a confirmation letter and/or email will indicate if an award will be granted. The confirmation letter and/or email will be sent prior to the due date of the first training fee. Applications must be submitted, reviewed and approved annually and are not automatically renewable. Please note that any awards will also contain 10 mandatory volunteer hours.
